This page provides the information and resources you need to complete the Relapse Prevention Planning course. You can view or download the handout, view the video overview of the presentation, and submit your brief plan once complete. The handout is a sample comprehensive relapse prevention plan. You can use that as a guide in preparation for submitting a brief plan (which you will do using the button at the bottom of the page). When you are ready, use the button below to submit your brief plan. Once you complete this and submit it, you will receive an email with your responses immediately. Your responses will be sent to Sober2Day, and a written plan will be provided to you within 24 hours along with a certificate of completion. These will also be sent to your attorney if applicable.
